Winter 2026 orientation has passed. For continued interest, please apply when applications open again in May.

Getting started with MCH

Download and fill out all of the forms appropriately. Rename all forms as indicated.

To indicate your availability, please note that for the Winter 2026 session, volunteering shifts are on Sundays and Mondays at the following times:

  • Morning: 10:30 AM – 12:30 PM

  • Afternoon: 1:30 PM – 5:30 PM

  • Evening: 5:30 PM – 6:30 PM

  1. Montreal Chinese Hospital Volunteer Form

  2. CIUSSS Agreement Form

  3. CIUSSS Volunteer Recommendation Letter (No friends or family members. We do not require the submission of letters — please just fill out the given template.)

  4. Criminal Record Checkup Consent Form

Take a picture/scan evidence of immunization for COVID-19 (dose 1, dose 2, and a booster shot). We accept .jpg, .jpeg, .png, .pdf, .doc, and .docx file formats. (Please do not submit any .zip or .page files!)

Send digital copies of all completed forms and proof of vaccination to Andrea Lai at [email protected], Kavid Kwan at [email protected], and your school executives.
For McGill students, this will be [email protected], and for Marianopolis students, this will be [email protected].

The deadline for your completed forms is your respective orientation day. The deadline for recommendation letters is a week after orientation day.
Orientation for Winter 2026 will be on February 15, 2026.

Print a copy of all of your forms, and bring them to orientation day!

Orientation will take place at the Montreal Chinese Hospital (189 Avenue Viger E, Montréal, QC H2X 3Y9).
